This form is used by prospective combined degree students who plan to pursue two degrees—one through the Graduate School and one through either a professional or undergraduate college (e.g., BS/MS, MD/MPH, JD/MBA). Combined programs offer exceptional students a streamlined pathway to earning two degrees concurrently.

Important Notes
Students must first submit an official application through the Graduate & Professional Admissions portal. To be admitted, applicants must provide documentation showing they meet the following criteria:
Undergraduate/Graduate Combined Programs
- Completion of at least 90 undergraduate credit hours.
- Minimum 3.5 cumulative GPA across all undergraduate coursework (including transfer credits).
Professional/Graduate Combined Programs
- Either a completed bachelor’s degree or completion of the first year of a professional program.
- If holding a bachelor’s degree but not one year of professional study: minimum 3.5 cumulative undergraduate GPA (including transfer credits).
- If one year of professional study is completed: must be in good academic standing, admitted to the Graduate School without conditions.
All Applicants
- Must be in good academic standing, admitted to the Graduate School without conditions, and approved by the professional college or undergraduate program.
- Prerequisite training appropriate for the intended graduate program.
Students cannot be admitted to a combined degree program in conditional or graduate non-degree status.
Note: This form is not intended for students pursuing two graduate degrees. Those students should instead submit the Dual Degree Program Plan Form.
Submission Instructions
Step 1: Log in to gradforms.osu.edu and go to your Enrollment page.
- Select "Forms" → “Enrollment."
Step 2: Click "CREATE NEW+ " next to ‘Combined Degree Program Plan’.
- Alternatively, click on “Combined Degree Program Plan,” which directs you to a new page. Click on “+ CREATE COMBINED DEGREE PROGRAM PLAN FORM."
Step 3: Complete the form by entering:
- Your current career
- (e.g., undergraduate or professional program)
- Your proposed program
- Confirm submission of official application
- Matriculation Term
- The term you plan to begin the graduate program
- Cumulative Undergraduate GPA
- Courses you plan to double count
- List only the courses you intend to apply toward both degrees.
- If you are not double counting any courses, enter:
- “None” for the Department, any term for the Term, and “0000” for the Course Number, and “0” for the Credit Hours field.
Step 4: Review all information carefully before clicking “Submit."
- After submission, you cannot edit the form. To request changes, leave a comment on the form or contact your advisor.
To leave a comment, go to “Forms” → “Enrollment” → “Combined Degree Program Plan."
Find your form, click “View” → “Comments."
* Comments are visible to both the student and faculty/staff.
- If you are not ready to submit, click “Save” to keep your draft.
- To return to a Draft:
Go to “Forms” → “Enrollment” → “Combined Degree Program Plan."
* You can use this same pathway to check the status of your form after submission.
Find your Draft, click “View”, then select “Review and Submit."
Note: The “Combined Degree” checklist item in your Application Portal will remain until your form is fully approved by all parties, including the Graduate School.
Approval Workflow
- Once submitted, the form is manually routed to the appropriate graduate and undergraduate/professional college approvers for “Initial Review."
- Note: Your form will initially appear as “Grad School Review” before it is sent to the designated approvers.
- After all designated program approvers have posted a decision, the form is routed to the Graduate School for “Grad School Review” and approval or denial.
- If approved, the Graduate School posts the admission decision.
- The Graduate School will mark your form “Complete” upon graduation.
Important Notes for Programs and Colleges
- Please ensure all application requirements have been completed by the student and in the Student Information System before approving their form.
- Courses approved for double counting will be added to the student’s graduate record after they complete their undergraduate degree. If the student plans to complete both degrees in the same term, please email grad-schoolregistrationservices@osu.edu.
- The student will receive an admission notification email from the Office of Graduate and Professional Admissions. This email notification includes instructions on how to accept the admission offer through the student’s Buckeye Link page.
- Combined students must maintain minimum enrollment requirements for both programs, which may impact tuition and financial aid.
